Have your vision or medical insurance card ready at check-in. We'll verify your benefits before your appointment.
Bring a list of current medications, including eye drops. Some medications can affect your vision and eye health.
New patients should arrive 10–15 minutes early to complete intake paperwork, or download and fill out forms in advance.
